We are preferred and participating provider for many insurances

Please Bring your insurance card with you on the day of your visit

OHP; we see Family care insured patients who reside from Echo, through Pendleton, and as far east as Adams. We do not see Care Oregon patients. OMAP insured patients are seen on a case-by-case basis at the discretion of the physicians.

Disability Insurance

Our office is very willing to complete your disability insurance forms. You can help us by filling out your portion of the form and either mail or bring them to the office. We try to have these completed within five working days. We need to know if they are to be sent back to you, or on to the insurance company. There is a $10.00 charge per form completion.

We also have on-site X-Ray facilities

We perform sports physicals on a case-by-case basis

Neck and Back problems are treated on a case-by-case basis at the discretion of the physicians.

We do not perform impairment ratings, or disability exams.

Dr. Weeks performs independent medical exams (IME's) and arbiter exams.

We do not see out of state on-the-job claims cases.

Please bring your insurance card with you on the day of your visit. As a courtesy, we will gladly bill your insurance company directly for services rendered. In order for our office to provide this service to you, we must have complete insurance information. All copays are due on the day of your visit. We accept MasterCard and Visa.

Patients whose insurance requires a referral must obtain a referral from their primary care physician before the day of their appointment, or the appointment will be rescheduled.
