Our office is very willing
to complete your disability insurance forms. You can help us by filling
out your portion of the form and either mail or bring them to the
office. We try to have these completed within five working days. We need
to know if they are to be sent back to you, or on to the insurance
company. There is a $10.00 charge per form completion.
